nl: KPN update

This patch updates the following
- make kpn a primary provider (they have a lot of other providers on their network)
- adjust prepaidinternet apn to be plan type prepaid
- add KPN4G.nl apn
- add (legacy) portalmmm.nl apn
- add (legacy) portalmmm.nl mms gateway
