menus_and_toolbars: Replace UIManager with Builder.

* examples/book/menus_and_toolbars/examplewindow.[h|cc]:
  Because UIManager (and GtkAction) is deprecated.
  However, we still need to add the toolbar back somehow,
  and the accelerators do not work.
